Clinical Medicine and Professional Features

As a national key clinical specialty and the leading unit of endocrinology in Shandong, the Department of Endocrinology is at the forefront of the country in the diagnosis and treatment of diabetes, obesity, thyroid disease, bone metabolic disease, adrenal disease and pituitary disease. Professional diagnosis and treatment of rare diseases and various acute and critical diseases are also at the leading level in the province. The main professional features as follows:

The comprehensive diabetes management platform: the prevention and treatment of diabetes and complications is the focus of the development of the department, and a comprehensive diabetes management platform has been gradually established. The department and the community have carried out screening and counterpart support for the incidence of diabetes, and provided training on the prevention and treatment of diabetes and its complications for primary medical workers. Early diabetes risk assessment and complications screening were carried out in the outpatient clinic, and the electronic medical record follow-up system for diabetic patients was launched. The department has established a diabetes education clinic in the outpatient department to strengthen the management and education of outpatient diabetes patients. In order to better serve and manage Jinan medical insurance patients with diabetes, the department has established a diabetes follow-up clinic in the medical insurance department to provide more professional and detailed diabetes management, which is well received by patients. As the leader of the Regenerative Science Group of the Diabetes Branch of the Chinese Medical Association, Professor Chen Li actively explores the regenerative treatment of diabetes.

We have carried out diabetes stem cell treatment simultaneously with foreign countries, and achieved good results, creating a precedent for the treatment of diabetes by autologous bone marrow stem cell transplantation in our province and even the whole country, filling the gap in China, and opening up a new way for the research on the pathogenesis and treatment of diabetes. Preclinical research on umbilical cord mesenchyme combined with sequential infusion of stem cells, immune intervention in the treatment of type 1 diabetes and thyroid-related ophthalmopathy. The department has established a diabetic foot treatment room in the ward, equipped with advanced diabetic foot diagnosis and treatment systems, including blood flow tester, transcutaneous oxygen partial pressure tester, negative pressure therapy device, ultrasonic debridement device and other instruments, and actively cooperate with burn surgery, in collaboration with the Department of Vascular Surgery, comprehensive diagnosis and treatment of diabetic foot were carried out, and the Diabetic Foot Diagnosis and Treatment Center of Qilu Hospital of Shandong University was planned. Diabetes patient health education plays a key role in the management of diabetes. Departments have diabetes education courses in outpatient clinics and wards every day, and diabetes lectures are held in the hospital every month. In 2013, our department was rated as a national diabetes education demonstration unit. The department organized and carried out diabetes specialist nursing training, which is the province's diabetes specialist nursing training base.

The characteristics of thyroid disease prevention and control research are obvious: in recent years, the incidence of thyroid disease, especially thyroid cancer, has shown a rapid growth trend. The department has formed a relatively complete clinical diagnosis and treatment system composed of thyroid clinic, ward, thyroid fine needle puncture room and radionuclide treatment room. In 2015, on the basis of the original characteristic technology, the department of pathology innovatively carried out ultrasound-guided fine-needle aspiration cytological examinations of the thyroid. So far, more than 12,000 punctures have been completed, and the compliance rate of malignant pathology has reached more than 95%, which is greatly improved the proportion of malignant tumors for thyroid surgery in our hospital. According to the Bethesda system, 20%-25% of thyroid nodules are diagnosed with FNA with uncertain nature, and molecular testing may make up for the lack of cytological diagnosis, guide preoperative risk stratification  and optimize diagnosis and treatment plans.

After the unremitting efforts of the Department of Endocrinology and Pathology, the BRAF gene detection of thyroid fine-needle aspiration cells was officially launched in May this year, filling the gap in the preoperative molecular detection of thyroid nodules in our hospital. At present, our hospital has routinely carried out thyroid fine-needle aspiration cell BRAF, TERT, HRAS and other genetic testing, which has improved the diagnosis and treatment system of thyroid nodules and further improved the diagnosis and treatment of thyroid diseases in our hospital. The department has done a lot of work in the radiotherapy of thyroid diseases, carried out thyroid iodine uptake rate inspection, and purchased advanced equipment such as automatic iodine analyzer to improve the efficiency and safety of treatment. Currently, more than 5,000 patients and more than 1,000 iodine-131 treatments has been received. The department has established an integrated electronic follow-up and medical record system for patients with thyroid diseases to further improve the diagnosis and treatment of thyroid diseases. The smooth development of the above work has led to an overall improvement in the diagnosis and treatment level of thyroid diseases in our hospital.

Obesity prevention and control has become an important branch of the development of the subject: a lot of work has been done in the field of obesity prevention and control, especially in the field of obesity prevention and control of adolescents, and the first obesity prevention and control center has been established in the province. The department has established an obesity clinic, and carried out body fat composition monitoring, electrical impedance and dual-energy X-ray absorption methods to determine the fat content and distribution of obesity, sleep breathing disorder monitoring; Epworth sleepiness score and Berlin questionnaire survey evaluation and RESMED Portable sleep monitor ensures early diagnosis and treatment of obstructive sleep apnea syndrome. The department cooperated with general surgery to carry out surgical treatment of obesity, established a weight loss and diabetes surgery clinic in Huamei Building, and established a weight loss and diabetes surgery expert team. The team is composed of professors Chen Li and Jiang Ling of the Department of Endocrinology and Hu Sanyuan of the Department of General Surgery and other well-known endocrinologists and minimally invasive surgery experts in and outside the province. The expert group compiled the "Shandong University Qilu Hospital for Weight Loss and Diabetes Surgical Treatment Manual". This action will continue to promote clinical and basic research in the field of bariatric surgery in the treatment of obesity and type 2 diabetes in our hospital, and benefit the majority of obesity and diabetes patients.

Osteoporosis prevention and treatment is fruitful: Since the early 1990s, the department has been the first to carry out research on osteoporosis in the province and gradually applied it in clinical prevention and treatment, with ultrasound bone densitometer and dual-energy X-ray bone densitometer. This year, we purchased the most advanced new generation dual-energy X-ray bone densitometer. It has a high level of diagnosis and treatment of metabolic bone disease.

Actively promote multidisciplinary collaboration: with the strong support of brother departments, we set up multiple MDT teams such as weight loss and metabolism, thyroid disease, pituitary disease, adrenal disease, etc., and regularly conduct MDT academic seminars, and new technologies such as ultrasound-guided fine-needle aspiration cytology, adrenal vein blood sampling, and hypopituitar sinus blood sampling have been jointly carried out to fill the gap in hospitals and greatly improve the level of diagnosis and treatment of related diseases.
